On macOS, explicit credential paths (--creds-path, credentials_path, named accounts) no longer fall back to the Keychain, and a stale zeroed credentials file at the default path now does.

What changed in v0.12.0

Declare accounts under [[anthropic.accounts]] (label, credentials_path) and pick one with --account <label>; each has its own cache, and the TUI shows a tab per account. --cache-dir and --creds-path became documented flags. Model-scoped weekly limits such as the Fable weekly cap now show in the tooltip, the TUI and the bar's severity. Groups v0.9.0 to v0.12.0.
